How to Refer a Family to Chrysalis

We’re always happy to hear from professionals or agencies working with families who may benefit from our support.

To move forward with a placement, a formal referral must be completed by the Local Authority Social Worker using our Chrysalis referral form.

What is the referral process?

  • Initial contact to Chrysalis from agency/Social Worker.
  • Chrysalis will provide the agency/Social Worker with our e-brochure.
  • A referral form will be submitted by the agency Social Worker.
  • A telephone discussion unique to each specific parent will take place.
  • Chrysalis Assessment and Support Centre will carry out a Viability Assessment.

Welcoming families with care & support

Once a placement is accepted, we’ll send our Secure Placement Form and Terms & Conditions to the assigned Social Worker for review and signature by a representative of the Local Authority.

As soon as the referring agency confirms funding, we’ll secure the booking and send written confirmation.

The parent and child will then move into our Centre, marking the start of their programme.

Within 72 hours of their arrival, we’ll hold a Placement Planning Meeting. This will include the parent, the referrer, our Placement Managers, Link Workers, and other key professionals. Together, we’ll outline the support and assessment package. During this meeting, we’ll also discuss the level of supervision needed in the early stages of the assessment.
