To move forward with a placement, a formal referral must be completed by the Local Authority Social Worker using our Chrysalis referral form.
What is the referral process?
Once a placement is accepted, we’ll send our Secure Placement Form and Terms & Conditions to the assigned Social Worker for review and signature by a representative of the Local Authority.
As soon as the referring agency confirms funding, we’ll secure the booking and send written confirmation.
The parent and child will then move into our Centre, marking the start of their programme.
Within 72 hours of their arrival, we’ll hold a Placement Planning Meeting. This will include the parent, the referrer, our Placement Managers, Link Workers, and other key professionals. Together, we’ll outline the support and assessment package. During this meeting, we’ll also discuss the level of supervision needed in the early stages of the assessment.
